The root of all evil.

Because my dwarf ranger was swallowed by it and had to cut his way out of what was described to us as a "giant demonic parsnip" and that is possibly the most badass deed I've ever accomplished in D&D.

Also like the turlemoi from MM V in 3.5. Fun monsters, the more you damaged them the stronger they got. A pain to run though.

I once had a ghost of a giant octopus pursue the PCs through an entire sunken castle, phasing through walls, casting drowning enchantments on them, getting multiattacks from the arms. If they figured out ways to harm ghosts, he would eject "ghost ink" (basically steam fog) and leave them alone for a few hours, but eventually he came back to hassle them some more, usually at inopportune times like when you were already in combat with some merfolk soldiers.

So I'm gonna say octopus ghosts.

It was actually pretty fun. My dad's rogue got a cri with telling blow on the druid boss and killed him with ONE HIT. This was a halfling rogue literally charging the fucker with a dagger when he had 11 Strength. Hilarious

Pandorym, Elder Evil.
I never liked the Far Realm as a representative of "this is so far outside your reality you cannot even comprehend it". The Far Realm is just lovecraft tentacle cthulhu land. Pandorym is a more interesting monster in that it has no appearance. Its body exists, but looking at it gives you no information because it's not even conceivable.

Far Realm is so much worse when done right.

The known worlds are made up of the elements and the positive and negative energy planes. The Far Realm is made of of two elementas - Mind and Flesh. Flesh is influenced by mind, and mind is constantly generating random minds everywhere. when one actually survives long enough to decide it is a living thing, then it manipulates flesh into forming a body, and that is how anything that comes from the Far Realm is....born.
